Summary
Independence Blue Cross has removed 8 CPT codes (C5271-C5278) from the MA11.078e Scar Revision policy effective 01/01/2026. These codes are no longer valid for billing scar revision procedures under Medicare Advantage plans. The underlying coverage criteria for scar revision remain unchanged—procedures are covered when medically necessary (functional impairment, chronic pain, or part of reconstructive treatment plan) but excluded when purely cosmetic.
Action Required
By December 15, 2025: Billing team must update billing system to remove or inactivate CPT codes C5271, C5272, C5273, C5274, C5275, C5276, C5277, and C5278 from the Independence Blue Cross Medicare Advantage scar revision policy. Any claims submitted with these 8 codes on or after 01/01/2026 will be denied. Review any pending or staged claims using these codes and resubmit with valid alternative codes (such as 0479T, 0480T, or 11400-11401 as appropriate based on procedure type) or contact IBX for guidance on correct coding. Update internal billing guidance and provider notices to communicate the code deletion. Verify that all patient scheduling and encounter documentation systems reflect the change to prevent future claim rejections.
