Action Required

Update CPT code 62287 billing descriptions to reflect revised narrative from the policy. Configure system to reject and deny all claims containing codes 62330, 62331, or any percutaneous discectomy codes for Medicare Advantage plans. Verify that CPT 0274T displays updated narrative in billing software. Notify providers and front desk staff that percutaneous discectomy procedures (all codes) are not covered under Independence Blue Cross Medicare Advantage plans and should not be scheduled without patient financial counseling and informed consent forms documenting non-coverage.
