Summary
Independence Blue Cross removed two HCPCS drugs (J0288 amphotericin b and J1457 gallium nitrate) from the MA05.060e External Infusion Pumps policy effective 01/01/2026. These drugs are no longer covered for infusion pump administration under this policy. The policy itself remains substantively unchanged for other indications (chronic iron overload, cancer, intractable pain, and diabetes).
Action Required
By January 1, 2026: Billing team must update claim submission rules and billing software to deny or reject claims for J0288 (amphotericin b cholesteryl sulfate complex) and J1457 (gallium nitrate) when billed with external infusion pump codes (E0779, E0780, E0781, E0784, E0787) for Independence Blue Cross Medicare Advantage members. Add a system alert or hard stop to prevent submission of these drug-pump combinations. Notify providers and clinical staff that these drugs are no longer covered under the infusion pump policy; providers may still bill for the drugs separately if medically necessary under other policy criteria. No action needed for E0779, E0780, E0781, E0784, or E0787 themselves, as these pump codes remain covered for approved indications.
