Summary
Independence Blue Cross Medicare Advantage (MA) updated policy MA11.051c effective January 1, 2026, reclassifying single-anastomosis duodeno-ileal bypass with sleeve gastrectomy (SADI-S/SIPS) from Experimental/Investigational to Medically Necessary, making it now a covered bariatric procedure. By December 15, 2025: Billing team must update billing software and prior authorization systems to reflect SADI-S/SIPS (single-anastomosis duodeno-ileal bypass with sleeve gastrectomy) as a Medically Necessary covered procedure under Independence Blue Cross Medicare Advantage plans, removing it from experimental/investigational status. Update claim processing rules, EMR templates, and prior auth workflows to recognize CPT codes 43644, 43645, 43770, 43771, 43772, 43773 and HCPCS code S2083 as covered services when medical necessity criteria are met (BMI ≥35 kg/m² or ≥32.5 for Asian patients, documented comorbidity, failed medical treatment history). Add CPT code C9785 (endoscopic outlet reduction) to experimental/investigational tracking. Providers should document that candidates meet all four required criteria before submission. Failure to update systems before the January 1, 2026 effective date will result in improper claim denials for SADI-S/SIPS procedures and payment delays for affected patients.
