DELETIONS: Remove 23 terminated codes from allowed/billable code lists (0033U, 0131U, 0132U, 0135U, 0544U, 0550U, 0551U, 0042T, 0394T, G6003-G6017). REVISIONS: Update code narratives/descriptions in system for A4351, A4352, 83015, 83018, 77402, 77407, 77412. Verify that claims using terminated codes will deny with appropriate denial messaging. Update Attachments A1, A2, B2.1, B2.2, B3, C1, and C2 in practice management system.
