Summary
Independence Blue Cross Medicare Advantage policy MA11.015ag has been superseded by MA11.015ah, effective 04/01/2026. This update adds 27 new HCPCS codes (A2040-A2045, G0681-G0684, Q4418-Q4440) for bioengineered skin substitutes and revises narratives for codes A2014 and A2037. Billing teams must update systems to recognize these new codes for wound care claims.

By March 31, 2026: Billing team must update billing software and claim submission systems to recognize the 27 newly added HCPCS codes (A2040-A2045, G0681-G0684, Q4418-Q4440) as medically necessary for lower extremity chronic non-healing wounds, burns, acute wounds, post-Mohs, and post-surgical use under Medicare Advantage. Concurrently update code narratives for A2014 and A2037 per policy MA11.015ah. Verify that prior authorization rules, medical necessity edits, and coverage criteria align with updated policy requirements (wound size ≥1.0 cm², failed response to conservative care ≥4-30 days depending on wound type, adequate circulation). Remove all references to superseded policy MA11.015ag from internal resources and provider communication. Test claim submissions with new codes before April 1, 2026 effective date. Claims submitted with newly added codes on or after 04/01/2026 will be denied if system is not updated.
