By April 6, 2026 (policy publication date): Billing team must immediately update billing system rules and coverage edits for Independence Blue Cross Medicare Advantage to: (1) Add new HCPCS codes A4295, A4296, and A4297 as medically necessary for intermittent catheterization; (2) Remove face-to-face visit requirements from claims adjudication logic for A4342 and A4352 to prevent automatic denials; (3) Expand medical necessity criteria to cover all individuals with spinal cord injury for intermittent catheterization (not limited to pregnant females); (4) Update claim scrubbers to reflect revised code descriptions for A4351 and A4352 now including hydrophilic coating options; (5) Providers should document spinal cord injury diagnosis (ICD-10 codes starting with S14) on claims for these supplies. Retroactive effective date of January 1, 2026 means claims from that date forward must reflect new coverage rules. Failure to update system rules will result in inappropriate denials for covered supplies.
