Specifically: (1) For L1832, L1833, L1850 orthoses—require documentation of objective knee instability with physical examination findings including specific joint laxity tests (varus/valgus instability, anterior/posterior drawer test); (2) For L1843, L1845, L1851, L1852 orthoses—require either objective knee instability documentation, recent injury/surgery documentation, or osteoarthritis diagnosis with documented ambulatory status, pain, and functional reduction; (3) Update prior authorization templates and denial reasons in billing system to enforce these documentation requirements. Communicate updated requirements to providers and DME suppliers. Claims lacking required objective examination documentation will be denied retroactively to 01/25/2026. Update encounter/authorization forms to include checkboxes for required physical examination tests.
