Summary
Independence Blue Cross MA05.007g adds coverage for two new medications via small-volume nonfiltered nebulizers: hypertonic saline (for CF, bronchiectasis, ciliary dyskinesia) and ensifentrine (as add-on COPD therapy for patients on dual/triple maintenance therapy with continued exacerbations or dyspnea). Two new HCPCS codes (J7131, J7601) are added to support billing for these inhalation solutions.
Action Required
By January 31, 2026: Billing team must update system to recognize HCPCS codes J7131 (hypertonic saline, 1 ml) and J7601 (ensifentrine, 3 mg unit dose) as covered medically necessary drugs when administered via small-volume nonfiltered nebulizer. Providers must document specific diagnoses (CF/bronchiectasis/ciliary dyskinesia for J7131; COPD exacerbations or dyspnea for J7601 with dual/triple therapy documentation). Update encounter templates to capture ensifentrine patient criteria (LABA/LAMA or LABA/LAMA/ICS therapy status). Claims submitted with these codes on or after February 1, 2026 must include supporting diagnosis codes and therapy history or will be denied for medical necessity.
