(1) Providers must document the E/M service as distinct and separately identifiable from the same-day procedure in clinical notes. (2) Billing staff must verify that E/M codes (99201-99215 range) are appended with Modifier 25 only when documentation supports a significant, separate service. (3) Update billing system and encounter templates to include modifier 25 guidance and medical necessity prompts. (4) Train all clinical and billing staff on proper application of Modifier 25 per this policy effective date. Failure to properly apply Modifier 25 per policy requirements may result in claim denials or coding audits.
