Summary
Independence Administrators has reissued policy 06.02.36d with a critical disclaimer change: molecular testing (PathFinderTG®, PancraGEN®, BarreGEN®) is now NOT COVERED for members whose groups have enrolled in the UM vendor program, effective upon their renewal date. The policy remains experimental/investigational and non-covered, but applicability is now limited to groups that have NOT enrolled in the UM vendor program. Billing teams must verify member enrollment status before processing claims.
Action Required
By June 24, 2026, billing team must: (1) Update claim submission system to verify whether each member's group has enrolled in the Independence UM vendor program before allowing billing of CPT 84999 or 89240 for molecular testing (PathFinderTG®, PancraGEN®, BarreGEN®); (2) For members in groups that enrolled in UM vendor program, deny these claims and refer to eviCore Lab Management Program policy instead; (3) For members in groups that have NOT enrolled in UM vendor program, continue denying as experimental/investigational per existing policy; (4) Update billing software rejection rules and front desk intake procedures to include UM vendor program enrollment verification. Train all billing and collection staff on the new distinction. Failure to verify enrollment status may result in inappropriate claim submissions and denials.
