Action Required
By April 1, 2025: Billing team and providers must review and implement Modifier 25 guidelines for same-day E/M and procedure billing. (1) Train all billing staff and providers on when Modifier 25 is appropriate and required documentation standards for significant, separately identifiable E/M services. (2) Update billing software rules to flag claims with same-day E/M and procedure codes requiring Modifier 25 validation. (3) Add documentation reminder to EMR templates requiring providers to document the medical necessity and distinct nature of E/M services performed on procedure days. (4) Audit encounter forms and superbills to ensure Modifier 25 is available for selection when applicable. Failure to properly apply Modifier 25 or inadequate documentation of separate services will result in claim denials or reductions.
