Summary
Independence Blue Cross updated their Experimental/Investigational Services policy (12.01.01bu) effective 07/01/2025, adding 36 new CPT codes and 1 HCPCS code designated as experimental/investigational (not covered), removing 2 CPT codes from experimental status (now covered for peripheral nerve repair), removing 1 terminated HCPCS code, and consolidating 3 unlisted code services into separate policies. Billing teams must update system configurations to deny claims for the newly added codes and allow coverage for the removed codes.
Action Required
By June 30, 2025: Billing team must update IBX commercial billing system to: (1) Add CPT codes 0948T, 0949T, 0956T-0987T (excluding gaps), and 0558U, 0559U, 0568U, 0570U, 0573U, 0574U, plus HCPCS code Q4369 to the experimental/investigational exclusion list and configure system to deny all claims with these codes effective 07/01/2025; (2) REMOVE CPT codes 64912 and 64913 from experimental/investigational status and allow coverage for peripheral nerve injury repair using synthetic conduits or processed nerve allografts; (3) DELETE HCPCS code M0248 from all billing rules; (4) Remove unlisted codes 46999, 43499, and 43999 from experimental/investigational tracking since services are now reported under CPT 0963T or addressed in separate POEM policy 11.03.17; (5) Update system to reflect revised HCPCS code C8005 narrative effective 07/01/2025. Ensure denial reasons are sent to providers with policy reference 12.01.01bu. Test system changes before effective date to prevent claim processing errors. Failure to implement will result in improper denials or coverage of non-covered experimental services.
