Summary
Independence Blue Cross (IBX) has updated policy 07.02.21s to redefine coverage for continuous ambulatory electrocardiography (AECG) and mobile cardiac outpatient telemetry (MCOT) monitoring. Continuous AECG monitoring is now covered for periods up to 15 days (revised from previous criteria), while monitoring beyond 15 days is classified as experimental/investigational and not covered. Additionally, ICD-10 diagnosis code Z09 has been removed from the medically necessary list, effective July 21, 2025.
Action Required
By July 21, 2025: (1) Billing team must update billing system and claims submission rules to enforce the 15-day maximum limit for continuous AECG monitoring coverage. (2) Configure system to automatically deny or flag claims for continuous AECG monitoring beyond 15 days as experimental/investigational—these should not be submitted to IBX or require patient notification of non-coverage. (3) Remove ICD-10 diagnosis code Z09 (Encounter for follow-up examination after completed treatment for conditions other than malignant neoplasm) from the medically necessary diagnosis list for AECG/MCOT monitoring claims; do not use this code to establish medical necessity for this service. (4) Providers should be notified that authorization for AECG monitoring must specify the monitoring duration and confirm it does not exceed 15 days. (5) Update claim scrubbing logic to prevent billing for extended monitoring periods. Claims submitted beyond the 15-day threshold will be denied by IBX.
