By 10/20/2025: Billing team must update billing system to deny or flag claims for CPT code 0977T (upper gastrointestinal blood detection sensor capsule) as experimental/investigational and non-covered under Independence Blue Cross commercial plans. Update claim submission rules to route 0977T claims to a manual review or denial queue. Notify providers that CPT 0977T is no longer covered and patients may be billed directly or require alternative testing. Ensure that CPT codes 91110 (esophagus through ileum) and 91111 (esophagus only) continue to be processed as covered when medical necessity criteria are met (occult GI bleeding, small bowel neoplasm, Crohn disease, pre-surgical evaluation, esophageal varices). Claims for 0977T submitted without prior clarification of non-coverage may result in denials or payment recoupment.
