Summary
Independence Blue Cross has updated policy 05.00.21ai to clarify DME coverage criteria and added 41 HCPCS codes to Attachment B as non-covered items because they do not meet the company's definition of DME. Effective 11/10/2025, supplies represented by these codes (primarily wound care and ostomy supplies) will no longer be eligible for reimbursement consideration. Billing teams must immediately identify and stop billing these codes to avoid claim denials.

By 11/10/2025: Billing team must immediately audit claims history and billing workflows to identify all instances where the 41 newly non-covered HCPCS codes (A4212, A4337, A4461, A4483, A4860, A4932, A6219-A6224, A6228-A6233, A6266, A6402-A6404, A6407, A6413, A6441-A6456) are being billed to Independence Blue Cross Commercial plans. Update billing software to flag or block submission of these codes for IBC Commercial. Notify all providers and DME suppliers of the coverage change. Update internal claim submission checklists and encounter documentation templates to exclude these items from DME reimbursement requests. Establish process to deny or redirect any claims containing these codes submitted on or after 11/10/2025. Failure to comply will result in claim denials and potential compliance issues with IBC.
