Summary
Independence Blue Cross updated policy 12.01.01br to 12.01.01bu, effective 10/01/2025 (retroactively). The policy adds 11 new CPT codes, 1 existing CPT code, 4 new HCPCS codes, and 4 unlisted code services to the experimental/investigational exclusion list (not covered); simultaneously removes 1 CPT code from exclusions (now covered) and relocates 3 CPT codes and HCPCS codes to other policies. Billing teams must immediately identify and deny claims for newly excluded codes effective 10/01/2025.
Action Required
By 09/30/2025: (1) Billing team must update claims processing system to DENY all claims for CPT codes 0577U, 0579U, 0580U, 0581U, 0584U, 0589U, 0591U, 0594U, 0596U, 0598U, 0599U, and 0415U when dated 10/01/2025 or later, with denial reason: experimental/investigational service. (2) Billing team must update system to DENY claims for HCPCS codes C1740, C1742, C8006, and Q4391 when dated 10/01/2025 or later, with same denial reason. (3) Billing team must update denial rules for unlisted codes 38999, 43499, and 45399 when reported for lymphaticovenous bypass/anastomosis, Peroral Plication of the Esophagus (POPE), and Endoscopic Full Thickness Resection (EFTR) procedures effective 10/01/2025. (4) Reverse previous denials: CPT 43497 is now COVERED—update system to process claims as medically necessary; repricing may apply retroactively for dates 10/01/2025 forward. (5) Remove CPT 90901 (Freespira) from experimental/investigational edits; direct billing to policy 12.00.05 (Prescription Digital Therapeutics). (6) Remove CPT codes 57160, 59320, 59325 from experimental/investigational edits for Twin-Twin Transfusion Syndrome (TTTS) treatment; these are now medically necessary when properly documented. (7) Providers must be notified in writing of coverage changes, especially removal of TTTS and Freespira codes, to avoid unnecessary appeals. (8) Audit all denied claims from 10/01/2025 onward for these codes; rework any incorrectly processed claims. Failure to implement by effective date will result in coverage denials for newly excluded services and missed revenue recovery for newly covered services.
