Summary
Independence Blue Cross updated ICD-10 diagnosis codes for cobalamin, folic acid, and homocysteine testing effective October 1, 2025. Code E88.1 was deleted and replaced with six more specific codes (E88.10, E88.11, E88.12, E88.13, E88.14, E88.19) in Attachment A. Billing teams must update systems to use only the new codes when supporting claims for medically necessary B12 and folic acid testing.
Action Required
By October 1, 2025: Billing team must update coding system and billing software to DELETE ICD-10 diagnosis code E88.1 from all claim submission templates and coverage verification tools. ADD the six new diagnosis codes (E88.10, E88.11, E88.12, E88.13, E88.14, E88.19) to Attachment A reference materials and ensure they map to CPT codes 82607, 82608, 82746, and 82747 for cobalamin and folic acid testing claims. Update encounter documentation templates to reflect only the new codes. Audit any pending claims with E88.1 and resubmit with appropriate replacement code. Claims submitted with the deleted E88.1 code will be denied or require rework. Communicate the change to all ordering providers and clinical documentation staff to ensure correct diagnosis coding on requisitions.
