Summary
Independence Administrators updated the disclaimer language in policy 06.02.09g effective 03/04/2026 to clarify that HIV genotyping and phenotyping coverage now excludes groups enrolled in the UM vendor program. Billing teams must verify individual member benefits before providing services, as this policy no longer applies to members whose groups have opted into the UM vendor program upon their renewal date.

By March 4, 2026: Billing team must update member eligibility verification procedures to confirm whether each member's group is enrolled in the Independence Administrators UM vendor program. Before processing claims for HIV genotyping (CPT 87901, 87906) or phenotyping (CPT 87900, 87903, 87904) and interpretation (HCPCS G0452), verify in the billing system that the member's group has NOT enrolled in the UM vendor program. If a group has enrolled in the vendor program, route claims to the eviCore Lab Management Program instead. Update encounter forms and pre-service authorization workflows to include this verification step. Failure to verify enrollment status may result in claim denials or routing to incorrect payers.
