Summary
Independence Blue Cross updated policy 06.02.01l to 06.02.01m, effective 12/29/2025, adding PCR testing as medically necessary for Lyme disease diagnosis in immunodeficient individuals and for synovial fluid testing in Lyme arthritis. Five new HCPCS codes (S9497, S9500, S9501, S9502, S9503) for home infusion therapy at various dosing intervals were added as covered, CPT 87475 was reclassified from experimental to medically necessary, and CPT 0580U was added as experimental/investigational.
Action Required
By 12/29/2025: (1) Billing team must update billing system to recognize CPT 87475 as medically necessary (not experimental) for PCR direct probe detection of Borrelia burgdorferi, effective immediately upon policy implementation. (2) Add five new HCPCS codes (S9497, S9500, S9501, S9502, S9503) to the system as covered medically necessary home infusion therapy codes with appropriate documentation requirements for dosing frequency. (3) Flag CPT 0580U as experimental/investigational in billing system—claims using this code require prior authorization review and should be coded appropriately. (4) Update clinical documentation templates to capture PCR testing indications for immunodeficient patients and synovial fluid collection for suspected Lyme arthritis. (5) Provider staff should verify ICD-10 diagnosis codes (A69.20–A69.29) are properly linked to diagnostic and therapeutic codes to support medical necessity. Failure to implement these changes may result in claim denials or incorrect reimbursement for home infusion services.
