Summary
Independence Blue Cross updated policy 00.10.36v effective 01/01/2026 to reflect annual CPT/HCPCS code changes. Three new cardiac codes (33882, 33883, 33886) were added to the radiologic guidance policy, and eight codes (75842, 75956, 75957, 75958, 75959, 77014, G6001, G6002) were removed from coverage consideration. Billing teams must update system edits and code lists to reflect these additions and deletions.
Action Required
By 01/01/2026: Billing team must update claim editing systems and internal code lists to (1) ADD CPT codes 33882, 33883, and 33886 as eligible for separate radiologic guidance reimbursement when performed by the same provider who performs the procedure, with proper documentation; (2) REMOVE CPT codes 75842, 75956, 75957, 75958, 75959, 77014 and HCPCS codes G6001 and G6002 from radiologic guidance coverage consideration in Attachment A. Update billing software to deny claims using deleted codes in conjunction with radiologic guidance procedures. Notify providers that radiologic guidance for codes 33882, 33883, and 33886 now requires separate documentation of who performed the guidance service. Verify that claims submitted on or after 01/01/2026 reflect the updated code list. Failure to implement these changes will result in claims being billed incorrectly or denied.
