Action Required
By 01/01/2026: Billing team must remove HCPCS code J7309 from all PDT billing records, claim templates, and prior authorization forms. Update billing software to reject or flag any attempts to bill J7309 for photodynamic therapy claims. Verify that all active PDT claims use only J7308 (aminolevulinic acid HCl 20%, 354 mg) or J7345 (aminolevulinic acid HCl 10% gel, 10 mg) going forward. Notify providers and front-desk staff that J7309 is no longer billable effective 01/01/2026. Claims submitted with J7309 after this date will be denied.
