Summary

Action Required
By December 15, 2025: Billing team must update billing software and claim submission systems to recognize and properly process HCPCS codes G0568, G0569, and G0570 for Psychiatric Collaborative Care Management services effective January 1, 2026. Verify that all three new codes are mapped correctly in the system and associated with the existing CPT 99492 policy rules. Update provider training materials to educate clinicians on when to use the new HCPCS codes versus CPT 99492. Confirm that the CoCM program qualification requirements (written contracts, full-time behavioral health manager, weekly psychiatric consultation, electronic registry, validated outcome tracking) remain unchanged. Claims submitted with G0568, G0569, or G0570 without an approved CoCM program on file will be denied for lack of qualification.
