Action Required
By January 1, 2026: Billing team must remove CPT code 77014 from all billing systems, templates, and fee schedules for Independence Administrators claims. Update internal coding references and provider documentation to reflect that 77014 is no longer valid. Verify that proton beam radiation therapy claims use only valid CPT codes 77520, 77522, 77523, or 77525 for treatment delivery. Before processing any claims for proton beam therapy on or after 01/01/2026, confirm the member's group has not enrolled in the UM vendor program, as this policy does not apply to groups using external utilization management vendors. Claims submitted with CPT 77014 after the effective date will be denied.
