Summary
Independence Blue Cross updated CPT codes for percutaneous coronary intervention and coronary angiography procedures effective 01/01/2026. Six codes (92921, 92925, 92929, 92934, 92938, 92944) were removed, two codes (92930, 92945) were added, and six codes (92920, 92924, 92928, 92933, 92937, 92943) were revised. All procedures remain subject to Carelon Medical Benefits Management utilization review and clinical appropriateness guidelines.
Action Required
By December 15, 2025: Billing team must update billing system code tables to remove CPT codes 92921, 92925, 92929, 92934, 92938, and 92944 from all encounter forms, charge capture templates, and billing software. Add new CPT codes 92930 and 92945 to system and ensure these codes are available for selection. Update codes 92920, 92924, 92928, 92933, 92937, and 92943 in billing software to reflect any descriptor or bundling changes. Verify billing system is configured to route all affected codes (except emergent procedures) to Carelon Medical Benefits Management for prior authorization review. Test claims submission with new and revised codes in test environment before go-live. Claims submitted on or after 01/01/2026 using terminated codes (92921, 92925, 92929, 92934, 92938, 92944) will be denied. Notify interventional cardiologists and catheterization lab staff of code changes to ensure proper procedure documentation and coding selection.
