Summary
Independence Blue Cross reissued policy 08.00.51m with a HCPCS code deletion effective 01/01/2026: J0205 (alglucerase) has been removed due to market withdrawal. The policy maintains coverage for imiglucerase (J1786), taliglucerase alfa (J3060), and velaglucerase alfa (J3385) for Type 1 and non-neuropathic Type 3 Gaucher's disease. Billing teams must stop submitting claims with the deleted code immediately.
Action Required
By 01/01/2026: Billing team must remove HCPCS code J0205 (alglucerase) from all billing systems, claim submission software, and encounter templates. Code J0205 is no longer reimbursable due to market withdrawal and discontinued manufacture of Ceredase®. Verify that active coverage codes J1786 (imiglucerase), J3060 (taliglucerase alfa), and J3385 (velaglucerase alfa) remain in system with correct dosing and frequency parameters. Any claims submitted with J0205 on or after 01/01/2026 will be denied. Update internal documentation and provider alerts to reflect the code deletion. Notify prescribers if they are currently ordering Ceredase® to switch to one of the three covered alternatives.
