Summary

This is a reissue with no substantive changes to the AlloMap™ Molecular Expression Testing policy for heart transplant rejection. The only modification is disclaimer language clarifying that the policy applies only to members where Independence Administrators serves as claims administrator AND whose group has not enrolled in the UM vendor program; groups enrolled in the UM vendor program should refer to the eviCore Lab Management Program instead. Medical necessity criteria, covered codes, and benefit application remain unchanged.

Action Required

Action needed
By March 4, 2026: Billing team should verify whether your practice's employer groups have enrolled in Independence Administrators' UM vendor program. For groups that HAVE enrolled in the UM vendor program, stop using this policy (06.02.29g) for AlloMap™ testing and instead reference the eviCore Lab Management Program for coverage guidance. For groups that have NOT enrolled, continue using this policy as-is. Update internal policy files and billing system documentation to reflect the new disclaimer language. No changes to billing codes, medical necessity criteria, or prior authorization workflows are required.
