Summary
Independence Blue Cross reissued its autonomic nervous system (ANS) testing policy to reaffirm coverage criteria and clarify that noninvasive portable automated devices (ANSAR ANX 3.0, SUDOSCAN, electrochemical skin conductance) remain experimental/investigational and not covered. The BILLING REQUIREMENT header was removed, but coverage criteria for medically necessary laboratory-based ANS testing (CPT 95921, 95922, 95923) remain unchanged.

By June 16, 2025: Billing team must ensure claims for CPT 95921, 95922, and 95923 include medical necessity documentation demonstrating one of the covered indications (vasomotor instability evaluation, progressive autonomic neuropathy severity assessment, syncope differentiation, etc.). Verify that claims using noninvasive portable automated devices (ANSAR ANX 3.0, SUDOSCAN, electrochemical skin conductance testing) are denied as experimental/investigational and NOT billed to Independence Blue Cross commercial plans. Update billing software to flag and reject claims for ANS testing when used for screening only, disease monitoring without clinical status change, or acute motor axonal neuropathy diagnosis, as these remain non-covered. Providers should document clinical decision-making rationale in the medical record to support medical necessity before submitting claims.
