Summary
Independence Blue Cross expanded coverage for cleft lip/nose/palate repair by adding septoplasty and septorhinoplasty (CPT 30420, 30520) as medically necessary procedures when significant nasal function impairment is present. These procedures are now explicitly covered under this policy in addition to the existing dermabrasion/rhinoplasty policy. Billing teams must recognize these two new CPT codes as covered services for cleft-related cases effective March 23, 2026.

By March 23, 2026: Billing team must update billing system to recognize CPT 30420 (Rhinoplasty, primary; including major septal repair) and CPT 30520 (Septoplasty) as covered medical procedures under policy 11.03.01g when billed with cleft lip/nose/palate diagnoses (Q30.2, Q35.x, Q36.x, Q37.x) and when documentation supports significant nasal function impairment. Update claim edits and prior authorization rules to remove any denials for these codes in cleft repair cases. Verify that billing software correctly links these codes to the cleft palate diagnosis codes to ensure proper coverage determination. Notify providers that septoplasty/septorhinoplasty are now covered as part of cleft reconstructive plans. Confirm self-funded group benefit contracts separately—state mandates do not automatically apply to self-funded plans.
