Summary
Effective July 1, 2025, Molina Healthcare will begin immediate termination notifications for providers whose state licenses expire, with a 30-day grace period for renewal. Providers who renew within 30 days can be reinstated without re-credentialing, but those expired 31+ days must undergo full initial credentialing.
Action Required
By July 1, 2025: All providers must ensure state medical licenses are renewed before expiration dates. Set up license renewal tracking system and calendar reminders at least 60 days before expiration. If license expires, notify Molina immediately upon renewal within 30-day grace period to avoid full re-credentialing process. After 31 days expired, provider must complete initial credentialing before network reinstatement.
