Summary
Molina Healthcare of California is implementing Version 2 of the Housing Tenancy and Sustaining Services (HTSS) referral form with enhanced screening criteria for housing needs assessment. Starting June 11, 2025, referrals without required housing history, acuity, and clinical support documentation will not be processed.
Action Required
By June 11, 2025: Providers must begin using Version 2 of the HTSS referral form available at molinahealthcare.com/providers/ca/medicaid/forms/fuf.aspx. All HTSS referrals must include complete housing history, acuity levels, and clinical support needs documentation. Referrals submitted without this required information will be rejected and not processed.
