Summary
Molina Healthcare of California will now monitor practitioner licenses continuously and initiate immediate termination for expired licenses effective July 1, 2025. Providers have a 30-day grace period to renew without re-credentialing, but must go through full credentialing if license remains expired beyond 31 days.

Before July 1, 2025: All providers must ensure their medical licenses are renewed before expiration dates. Monitor license expiration dates closely as Molina will automatically terminate contracts for expired licenses. If license expires, renew within 30 days to avoid re-credentialing requirements. After 31 days of expired license, must complete full initial credentialing process before network reinstatement.
