Summary

Action Required
Immediately: Behavioral health providers must obtain signed Release of Information (ROI) forms from all patients and document refusals. Update intake processes to collect contact information for all other treating providers. Establish protocols to communicate with PCPs and other providers after initial assessment, when conditions change, during medication changes, and at treatment completion. Document all verbal and written communications with other care providers in patient records.
