Billing team should verify current coverage applicability with Aetna: (1) Confirm whether CPT 90651 (Gardasil 9, 9vHPV) claims are approved for patients ages 9–45 without prior three-dose Gardasil or Cervarix history; (2) Deny or request medical necessity review for CPT 90651 claims for patients who have completed a prior three-dose series, unless vaccine product history is unavailable (in which case continuation is medically necessary); (3) Ensure CPT codes 90649 (quadrivalent Gardasil) and 90650 (bivalent Cervarix) are marked as not covered, as these products are no longer distributed in the U.S.; (4) Flag claims with ICD-10 diagnoses A63.0, C44.520, C51.0–C53.9, D06.0–D07.2, D10.0–D10.9, D14.1, K62.82, or N87.0–N87.9 paired with HPV vaccine codes (90651, 90649, 90650) as experimental/investigational and apply non-coverage denial per policy exclusions; (5) Confirm plan benefit documents to verify whether preventive services (including HPV vaccination) are included in the specific plan design, as not all Aetna plans provide preventive coverage.
