Action Required

Before Mar 1, 2027
By March 1, 2027: Billing team should update billing software and workflows to require prior authorization from Anthem before submitting claims for CPT codes C9777 (esophageal mucosal integrity testing), E0767 (intrabuccal radiofrequency electromagnetic field device for cancer treatment), and J3386 (etuvetidigene autotemcel injection) for Georgia-based Anthem Medicare Advantage members. Front desk and clinical staff should implement precertification processes for these services. Providers may obtain detailed requirements at https://anthem.com/provider or through Availity.com for contracted providers. Failure to obtain prior authorization may result in claim denial.
