Summary

Independence Blue Cross Medicare Advantage updated medical policy MA11.019o effective 10/19/2026 to clarify that implantable Vagus Nerve Stimulation (VNS) for upper-limb impairment due to stroke is experimental/investigational and not covered. Implantable VNS remains covered only for medically refractory seizures; all other conditions including the newly documented stroke indication are non-covered. Billing teams must ensure claims for stroke-related VNS are denied and education provided to providers on non-coverage status.

Action Required

Action needed
Effective 2026-10-19: Billing team must update claim edits in the billing system to deny all claims for implantable VNS (CPT codes 0908T, 0909T, 0910T, 0911T, 0912T and associated HCPCS device codes C1767, C1778, C1816, C1820, C1822, C1883, C1897) when submitted with ICD-10 diagnosis codes indicating upper-limb impairment due to stroke. Educate in-network providers that implantable VNS for stroke-related upper-limb impairment is experimental/investigational and not a covered benefit under Independence Blue Cross Medicare Advantage plans. Confirm that implantable VNS remains covered only when billed with ICD-10 codes for medically refractory seizures (G40.011, G40.019, G40.111, and other intractable epilepsy codes). Claims submitted outside this approved indication will be automatically denied without prior authorization review. Flag any historical claims paid for VNS stroke treatment for potential recovery action.
