Action Required

Action needed
These policies became effective on June 1, 2026 and are now in effect as of October 6, 2026 (analysis date). The billing team must verify whether all four policy changes have been fully implemented in the practice's billing system and claim submission workflows: (1) Billing team should confirm that claims for MS-DRG 871 and 872 with length of stay <3 days and discharge to home are being denied or flagged for review before submission; (2) Billing team should verify that critical care codes 99291 and 99292 submitted for ED encounters with home discharge are being denied at the line level; (3) Billing team should ensure that all surgical procedure codes (CPT 10000–69999) that are bilateral-eligible have specific anatomical modifiers (RT, LT, E1–E4, F1–F9, T1–T9, LC, RC) and that generic modifiers (59, XS) are not substituted when a specific anatomical modifier applies; (4) Billing team should confirm that anesthesia claims on commercial plans no longer receive additional reimbursable units for physical status modifiers P3, P4, or P5 (modifiers may still be reported for documentation purposes only, with no additional payment). Confirm current applicability with Providence Health Plan and update internal billing guidelines, training materials, and claim validation rules accordingly. Claims submitted that do not comply with these policies will be denied.
