HISTORICAL MATERIAL – PAST EFFECTIVE DATES: This alert was published May 1, 2026 and discovered October 6, 2026. All stated effective dates (June 1 and July 1, 2026) have already elapsed. ACTION: (1) Contact Providence Health Plan directly to confirm whether these four policies remain in effect as of October 2026 and whether any superseding guidance has been issued. (2) If confirmed current, Billing Team should: For anesthesia claims (effective 6/1/2026, Commercial only): Remove additional unit claims associated with physical status modifiers P3, P4, P5 in billing system or claims scrubbing logic; retain modifiers for documentation only. For inpatient sepsis claims with MS-DRG 871 or 872 (effective 7/1/2026, Medicare and Commercial): Implement edit to deny claims when length of stay is less than 3 days AND discharge disposition is to home; educate providers to rebill under more appropriate DRG when clinical severity does not support assignment. For ED critical care claims with CPT 99291/99292 (effective 7/1/2026, Medicare and Commercial, facility billing only): Implement line-level denial edit when critical care codes are billed in ED with discharge to home (status code 01); recommend billing appropriate ED E/M level instead. For surgical procedures CPT 10000–69999 (effective 7/1/2026, all lines, professional and facility): Implement edit requiring correct anatomical modifiers (RT, LT, E1–E4, F1–F9, T1–T9, LC, RC) when bilateral-eligible or anatomically variable; deny claims missing, incorrect, or substituting non-specific modifiers (59, XS) when specific anatomical modifier applies. Consequences of inaction if policies remain current: claims will be denied at line or claim level, causing payment delays and requiring manual rebilling. (3) Retain this notice for compliance documentation, but do not implement any changes based on publication date alone—verification of current status is mandatory.
