Summary

Aetna updated its transabdominal cerclage policy (CPB 0529) to clarify medical necessity criteria and coverage limits. The procedure is covered only for specific indications (deep traumatized cervix, failed transvaginal cerclage, shortened/amputated cervix, or pre-conception placement) with documented incompetent cervix history. Prophylactic cerclage for multiple gestations remains experimental and non-covered.

Action Required

Action needed
Immediately: Billing team must update claim submission protocols for transabdominal cerclage procedures (CPT 57700, 59325). (1) Require providers to document one of the four medical necessity criteria: deep traumatized cervix, previous failed transvaginal cerclage, shortened cervix (<2.5 cm) or amputated cervix, or pre-conception placement via laparoscopy/laparotomy. (2) Ensure diagnosis codes submitted are limited to N88.3 or O34.30-O34.43; DO NOT submit O30.001-O30.93 (multiple gestation) codes as these indicate experimental/non-covered indications. (3) Implement internal pre-claim review to verify documented history of incompetent cervix (mid-trimester loss with painless cervical dilation, no uterine activity) is present in medical record before submission. (4) Flag any claims with multiple gestation diagnoses (O30.x codes) paired with cerclage codes for denial review, as these are not medically necessary per policy. (5) Update encounter templates to require providers to select from the four approved indications and document cervix trauma/failure history. Failure to verify medical necessity will result in claim denials.
