Summary
Aetna's Lead Testing policy (CPB 0553) clarifies medical necessity criteria for blood lead testing across three categories: diagnostic testing for symptomatic patients, occupational screening for workers, and preventive screening for high-risk children. The policy explicitly excludes coverage for lead measurement in bone, hair, teeth, or urine as experimental/unproven, and notes an FDA recall requiring retesting of certain populations previously tested with Magellan Diagnostics LeadCare analyzers.
Action Required
Billing team must immediately: (1) Verify CPT 83655 claims include appropriate ICD-10 diagnosis codes from the covered list (symptomatic lead poisoning indicators, occupational exposure codes Z57.2/Z57.4/Z57.8, or pediatric screening codes Z00.121/Z13); (2) Reject or deny claims for lead measurement in bone, hair, teeth, or urine (these are explicitly non-covered); (3) Flag claims for pregnant women without documented risk factors for lead exposure—routine screening in average-risk pregnant women is non-covered; (4) For pediatric lead screening, ensure documentation supports one of the high-risk criteria listed in policy (developmental delay, contaminated exposure, abuse/neglect, pica, contact with elevated lead level, immigration/adoption from endemic areas, iron deficiency, unexplained illness, high-prevalence community, pre-1950 or pre-1978 renovation-status housing, proximity to lead industry/traffic, household occupational/hobby lead exposure, ethnic remedies, or lead pottery use); (5) Identify patients previously tested with Magellan Diagnostics LeadCare Analyzer (before May 17, 2017) who were under 6 years old or pregnant/lactating with results <10 µg/dL and route for retesting per CDC recommendations. Update billing software edits to enforce covered diagnosis code requirements for CPT 83655.
