Summary

HMSA has issued new documentation requirements for providers referring members to the Medical Respite Program. All referrals must now include three specific completed forms (Medical Respite Authorization Form, CIS+ Member Consent Form, and Medical Respite Person-Centered Service Plan) before member admission. HMSA has created a fillable packet with a CIS+ program checklist to streamline submission of complete authorization requests.

Action Required

Action needed
Immediately: Providers referring members to Medical Respite Program must obtain and use HMSA's new fillable authorization packet. Update referral processes to ensure all three required documents (Appendix I: Medical Respite Authorization Form, Appendix C: CIS+ Member Consent Form, and Appendix J: Medical Respite Person-Centered Service Plan) are completed and submitted before member admission. Referral staff should access the fillable packet and CIS+ program checklist from the Provider Resource Center. Failure to submit complete documentation will delay or deny authorization for Medical Respite admissions.
