Summary

Aetna's CPB 0572 (reviewed 2026-09-01) establishes that home/ambulatory spirometry is medically necessary ONLY for lung transplant recipients (CPT 94014, 94015, 94016). All other indications—including asthma, cystic fibrosis, COPD, and interstitial lung disease—are classified as experimental/investigational and not covered. Billing teams must immediately deny or reject claims for home spirometry unless the patient has a lung transplant diagnosis (ICD-10 T86.810-T86.819 or Z94.2).

Action Required

Action needed
IMMEDIATE: Billing team must update claims processing system to DENY home spirometry claims (CPT 94014, 94015, 94016; HCPCS E0487) unless BOTH conditions are met: (1) Patient diagnosis is lung transplant status or lung transplant complication (ICD-10 Z94.2 or T86.810-T86.819), AND (2) CPT code is billed with one of these qualifying ICD-10 codes. Add hard-stop edit in billing software to reject claims with non-transplant diagnoses (asthma E84.0-E84.9, COPD J40-J94.9, cystic fibrosis, IPF, ILD, influenza/pneumonia J09.X1-J18.9, etc.) on CPT 94014/94015/94016 lines. Update encounter forms and provider education materials to clarify that home spirometry is NOT covered for asthma monitoring, COPD monitoring, or other pulmonary conditions—only for lung transplant recipients. Train front desk and billing staff to verify transplant diagnosis before claim submission. Any claims submitted without qualifying lung transplant ICD-10 codes will be denied by Aetna and will likely require patient appeals or write-offs.
