Summary

Aetna's CPB 0635 clarifies coverage for hematopoietic cell transplantation (HCT) in ovarian cancer, limiting medical necessity to autologous HCT for relapsed/remission germ cell tumors only. Epithelial ovarian cancers and allogeneic transplants remain experimental/non-covered. Billing teams must verify tumor histology (germ cell vs. epithelial) and remission status before authorizing claims.

Action Required

Action needed
Immediately: Billing team must implement prior authorization controls for HCT claims in ovarian cancer cases. (1) Require medical records review confirming diagnosis is GERM CELL tumor (not epithelial ovarian cancer) before authorizing CPT 38232 or 38241. (2) Deny or refer to medical director any claims for allogeneic HCT (CPT 38205, 38240) or autologous HCT for initial treatment/epithelial cancers. (3) Update billing software to flag ICD-10 codes C56.1-C56.9 and C57.4 with requirement for supporting documentation of remission status or relapse confirmation. (4) Provider education: Advise oncology practices that only autologous HCT for relapsed germ cell tumors responsive to chemotherapy, consolidation therapy in complete remission, or tandem transplant in relapse will be covered. (5) Claims submitted without tumor histology verification or for epithelial cancers will be denied as experimental/unproven. Include denial reason in EOB to reduce appeals.
