Summary
Aetna's facial prostheses policy (CPB 0620) was reviewed on September 3, 2026, establishing coverage for superficial facial prostheses and related supplies (adhesives, tape, removers) when there is loss or absence of facial tissue due to disease, trauma, surgery, or congenital defect. The policy specifies 19 covered CPT/HCPCS codes, excludes cosmetic skin care products, and clarifies that modifications, repairs, and follow-up visits within 90 days of delivery are included in the prosthesis allowance and not separately reimbursed.
Action Required
Immediately: Billing team must ensure all facial prosthesis claims are coded using only the approved CPT codes (21076-21088), HCPCS L-codes (L8040-L8049), and V-codes (V2623-V2629) listed in this policy. Update billing system to: (1) Bundle all modifications, repairs, and follow-up visits within 90 days of prosthesis delivery into the primary prosthesis charge—do not bill separately; (2) Reject and do not bill any claims for cosmetic skin care products (A6250, A6260) as these are excluded; (3) Allow separate billing for modifications occurring more than 90 days post-delivery only when medically necessary due to patient condition change. Educate providers and front desk staff that adhesives, tape, and removers (A4364, A4450, A4452, A4455, A4456) are covered only when medically necessary in conjunction with a facial prosthesis. Failure to bundle services correctly or to apply exclusions will result in claim denials and potential overpayment recovery.
