Summary
Aetna updated its Clinical Policy Bulletin 0592 on intranasal ablation procedures, clarifying coverage criteria for radiofrequency volumetric tissue reduction (RFVTR/Somnoplasty) of inferior turbinates for chronic nasal obstruction due to mucosal hypertrophy. The policy designates multiple ablation techniques and procedures as experimental/investigational, including coblation, cryoablation, RhinAer, temperature-controlled radiofrequency neurolysis, and NEUROMARK System. Billing teams must verify specific procedure type and diagnosis before claiming coverage under CPT 30801/30802, as many related codes and indications are explicitly non-covered.
Action Required
Immediately: Billing team must update claims submission logic in billing software to enforce strict coverage criteria for intranasal ablation procedures. (1) CPT 30801 and 30802 are covered ONLY for chronic nasal obstruction with inferior turbinate mucosal hypertrophy—verify diagnosis codes J34.0, J34.1, J34.81, J34.9, or J34.3 before submitting. Do NOT bill these codes for snoring (R06.83), allergic rhinitis (J30.1-J30.9), obstructive sleep apnea (G47.33), or rhinitis medicamentosa (J31.0, T48.5X5A-T48.5X5S). (2) CPT 31242 and 31243 (posterior nasal nerve ablation) are NOT covered under any indication. (3) Add hard-stop validation rule: if diagnosis code is allergic rhinitis, sleep apnea, or snoring, system must reject claims with CPT 30801/30802 and route to provider for rebilling with corrected diagnosis or procedure. (4) Flag all coblation, cryoablation, RhinAer (CPT 30117), temperature-controlled radiofrequency neurolysis, and NEUROMARK System procedures as experimental/non-covered—deny these claims automatically with explanation of experimental status. (5) Providers and coding staff must update encounter templates and pre-visit planning forms to capture accurate diagnosis. Update denial letter templates to cite CPB 0592 and specify non-covered indications. Monitor for appeal trends on denied claims and educate provider offices on coverage criteria. Failure to enforce these rules will result in claim denials post-adjudication and potential compliance issues.
