Summary
Aetna's updated policy (CPB 0525, reviewed 2026-08-18) clarifies coverage for lipid disorder screening, establishing medical necessity for specific cholesterol tests while classifying others as experimental/unproven. Key change: directly measured LDL-C is now covered for patients with triglycerides >250 mg/dL OR type 2 diabetes, but NOT for other populations. Billing teams must verify patient eligibility criteria before authorizing these tests to avoid claim denials.
Action Required
IMMEDIATE: Billing team must implement the following eligibility rules in billing system and encounter forms:
1. CPT 83721 (direct LDL-C measurement) - COVERAGE ALLOWED ONLY IF:
- Patient has triglyceride level >250 mg/dL, OR
- Patient has type 2 diabetes (ICD-10: E11.x)
Deny or request prior auth for all other scenarios (no direct LDL-C for low triglycerides or non-diabetic patients).
2. CPT 80061, 82465, 83718, 84478 (lipid panel components) - COVERED for screening and diagnosis when medically necessary; verify preventive vs. diagnostic coverage based on member plan.
3. CPT 83719 (direct VLDL-C measurement) - DO NOT BILL; mark as non-covered/experimental for all cases.
4. Verify ICD-10 diagnosis codes on claims: ensure E78.1, E78.2, Z13.220, or E08.00-E13.9 (diabetes) appear to justify testing.
5. Update claim denial protocols: automatically flag claims with CPT 83721 lacking documented triglyceride >250 mg/dL or diabetes diagnosis for correction before submission.
6. Train front desk and billing staff: Asymptomatic cholesterol screening may be non-covered for members without preventive benefits—always verify plan coverage before ordering.
Consequences of inaction: Claims will be denied for direct LDL-C testing in non-qualifying patients; preventive screening claims will deny if member plan lacks preventive coverage.
