Summary
Aetna has updated its Salivary Tests policy (CPB 0608, reviewed 2026-09-23) to clarify coverage criteria. Only late-night salivary cortisol for Cushing's syndrome diagnosis is covered; all other salivary tests are classified as experimental/investigational and not covered. Billing teams must immediately deny claims for non-approved salivary tests and ensure prior authorization documentation exists only for CPT 82530 and 82533 when used for Cushing's syndrome.
Action Required
Immediately: Billing team must update claims processing system to DENY all salivary test claims EXCEPT CPT 82530 (Cortisol; free) and CPT 82533 (Cortisol; total) when the diagnosis is E24.0-E24.9 (Cushing's syndrome). For all other indications listed in the policy as experimental/investigational—including adrenal insufficiency, depression, bipolar disorder, eating disorders, menopause, circadian rhythm disorders, cancer screening, celiac disease, and periodontal disease—automatically deny with explanation-of-benefits message: 'Salivary test not covered for this indication; considered experimental.' Update billing software to flag and block claims with ICD-10 codes E27.0-E27.9, F30-F34, F50, G47.20-G47.29, K90.0, K05-K13, M35.00-M35.09, N95.0-N95.9, and C codes when paired with salivary test CPT codes. Providers must document Cushing's syndrome diagnosis on all covered salivary cortisol orders. Stop accepting orders for CORE (oral fluid Rx monitoring), melatonin profiles, SARS-CoV-2 antibody saliva tests, and all cancer biomarker salivary tests. Contact providers in endocrinology, psychiatry, gastroenterology, and dentistry to clarify coverage restrictions. Failure to implement will result in claim denials and potential audit findings for unbundled or miscoded testing.
