Summary

UnitedHealthcare Community Plan of Pennsylvania revised its Oncology Medication Clinical Coverage policy, adding Vykoura (leucovorin) to the non-preferred oncology products list and introducing HCPCS code J0644 for billing. This policy governs coverage of injectable oncology medications based on NCCN Compendium evidence categories and requires prior authorization for non-preferred products when patients have documented intolerance to preferred alternatives.

Action Required

Before Nov 1, 2026
By October 31, 2026: (1) Billing team must update billing system and EMR to recognize HCPCS code J0644 for leucovorin (Vykoura) claims for Pennsylvania Medicaid members. (2) Reclassify Vykoura as non-preferred oncology product in system; route claims through prior authorization workflow requiring documentation of intolerance/contraindication to preferred leucovorin products. (3) Update prior authorization templates to require physician attestation that the same adverse event would not occur with Vykoura before approval. (4) Notify oncology providers that Vykoura claims will require prior auth effective November 1, 2026. (5) Flag any pending Vykoura claims under the old policy for reprocessing if needed. Claims submitted without prior authorization or without documented medical necessity (intolerance to preferred product) will be denied.
