Summary
UnitedHealthcare Community Plan of Nebraska is significantly narrowing coverage for TMJ disorder treatments effective December 1, 2026. The policy now covers ONLY TMJ arthroplasty as a surgical option (removing coverage for arthroscopy, discectomy, and reconstruction), eliminates four CPT codes, and removes the reference to botulinum toxin coverage for TMJ disorders. Billing teams must immediately identify and deny claims for discontinued procedures and update their coverage rules.
Action Required
By November 15, 2026: Billing team must update the UnitedHealthcare Nebraska Medicaid billing system to deny claims for CPT codes 21060, 21247, 29800, and 29804 (arthroscopy, discectomy, and reconstruction of TMJ). Configure system edits to automatically reject any claims for these codes submitted on or after December 1, 2026, with denial reason 'Not medically necessary per UHC policy effective 12/01/2026.' Simultaneously, update coding guidelines and provider communication to clarify that only TMJ arthroplasty is now covered for surgical treatment; all other TMJ surgical procedures are not covered. Remove any coverage references to botulinum toxin for TMJ disorders and direct providers to the separate Botulinum Toxins A and B drug policy if applicable. Notify affected providers (oral surgeons, maxillofacial surgeons, dentists, ENT specialists) of the change by November 20, 2026. Prior authorization personnel should update criteria to reference only InterQual® CP: Procedures: Arthroplasty, Temporomandibular Joint (TMJ) and remove outdated arthroscopy and reconstruction criteria. Failure to implement these changes will result in improper claim denials or inappropriate approvals.
