Summary
Humana Medicare Advantage revised its Peripheral Artery Revascularization of the Lower Extremities policy effective October 1, 2026, establishing specific medical necessity criteria for lower extremity PTA with stenting, atherectomy, and intravascular ultrasound. The policy references applicable Local Coverage Determinations (LCDs) by MAC jurisdiction and outlines coverage for alternative revascularization methods including deep vein arterialization and percutaneous transmural arterial bypass.
Action Required
By September 1, 2026: Billing team must implement updates for Humana Medicare Advantage claims processing to align with revised peripheral artery revascularization policy. REQUIREMENTS: (1) Verify member MAC jurisdiction (J5, J8, J6, JK, J15, JE, JF, JH, JL, or JN) in billing system to apply correct LCD requirements for CPT codes 37254-37299, 36245-36248, 37271-37278, 37288-37295, and 37252-37253. (2) Update prior authorization and medical necessity validation rules to require documentation of: Rutherford scale 4-6 for CLTI with atherectomy; in-stent restenosis status for atherectomy consideration; threatened bypass graft status; and objective arterial disease confirmation (ABI, TBI, TCPO2, or skin perfusion pressure testing) for CLTI cases. (3) Ensure providers document that guideline-directed medical therapy (exercise, lifestyle modification, pharmacologic therapy) was attempted before approving intervention codes. (4) Configure system to reference applicable MAC-specific LCD (L35998 for non-coronary vascular stents, L36767 for aortography/peripheral angiography, L40228 for endovascular management where applicable by jurisdiction). (5) Update encounter templates and authorization forms to include Rutherford classification and objective hemodynamic testing documentation requirements. Failure to implement these changes will result in claim denials for missing medical necessity documentation and potential overpayment recovery.
